Service Description

The DPC model is entirely focused on you with health and valuable time with your provider at its core. It is what health care should strive for to reduce unnecessary cost and promote actual health, well-being and education. It provides better health outcomes and personally benefits you--the consumer--while decreasing ER visits and hospitalizations.

Cancellation Policy

We understand that life happens and we try to accommodate everyone the best we can, but we need your assistance. 1. If you have an upcoming appointment and you found out that you will not be able to keep the appointment, please call or text us with 24-hour notice so we can fill the spot with someone who need health care and wasn't able to make an appointment prior. 2. If a patient cancels with less than 24-hour notice or no-show for their appointment, we will waive the first occurrence as a courtesy. 3. The second time a patient cancels within the 24-hour window of appointment, or a no-show, or late (past 10 minutes), there will be a charge of $25 fee prior to scheduling their next appointment. 4. If a patient is late more than 10 minutes past their appointment time, we will need to reschedule. Here at JCPC, we respect patient's time. This late encounter will count towards the same policy as a no-show or cancellation within 24-hour window (Fee is waived the first time). 5. A third no show, cancellation, or late appointment will grounds for dismissal from the practice. JCPC use an appointment reminder system that confirms appointments via phone call, text, and email 1 week, 36 hours, and 24 hours prior to your visit. If you are not receiving these reminders, please let us know ASAP so we can make sure we have the most current contact information.
